The Friedman Spiritual Awakening Scale has 6 subscales, each with 3 questions. Add up the sum of the score for each 3 subscale questions and place the number on the line.
1. True Nature (Questions 1 – 3)
2. Spirituality (Questions 4 – 6)
3. Higher Power / Inspired Action (Questions 7 – 9)
4. Awareness/Inclusion (Questions 10 – 12)
5. Perfection/Inner Unfolding (Questions 13-15)
6. Compassion, Empathy, Love (Questions 16-18)

Add up the scores for all 6 subscales to get the Total Friedman Spiritual Awakening Scale score and place that number on the bottom line next to Total Friedman Spiritual Awakening Scale Score.
